| Identify several interventions that might help improve response rates. Place them in order with the least-restrictive at the top so that the interventions may be introduced systematically, adding more restrictive / less favorable interventions after the outcomes of less restrictive options have been measured. | Provide training so that staff are clear on the expectation and can actually do it. Provide reinforcement for staff who take the binder when assigned, record data and submit it back at the end of their assignment. Corrective feedback and or discipline should be provided for failure to record data as assigned. |
