Stop any form of ignoring the behavior as this is the original trigger to self- injury. Provide non contingent attention to Tara then eventually work in appropriate ways to gain attention and reinforcement.

Give Tara 1:1 attention on a regular non contingent reinforcement schedule. Praise Tara when she is observed to engage in functional tasks and/ or positive behaviors on a VR-1 schedule. Teach Tara functional communication to get attention: tap the person's shoulder, give eye contact or say the person's name or action she needs e.g. "help me please" When she does this, the reinforcement is highly preferred to the attention she gets for hitting her head. Immediately eliminate the extinction procedure, the behavior is too dangerous to ignore.
