What are the ethical concerns?

The supervisee is not prioritizing her supervision.

How might you approach this situation if you were the supervisor?

Put the supervision expectations in writing and then terminate the supervisor relationship if there is not improvement.

According to the flowchart, the supervisor should submit the Alleged Violation Against an RBT.

If the supervisor was interested, and it was possible, another approach would be to cut the supervisees hours that month to be commensurate with the supervision she received. However, this could cause staffing problems and also might mean that a client would not receive services (which would be unethical for the supervisor).
