Lack of proper documentation or unorganized documentation can lead to problems in the future. As we say in education, "if we didn't see it, it didn't happen". This statement refers to data. If there is no data to present then the action can not be proven as actually happening.

The supervisor could spend some time modeling and training the supervisee on proper documentation and the importance of that data. One approach could be role playing a scenario where the supervisee has been asked to submit specific data, that they are not able to find/retrieve and also role play the consequences of that failure.

Fran's trainee presents risks due to persistent disorganization, problems with record-keeping, and problems with service delivery through lateness and inefficiency. These risks create problems for client well-being and create ethical and legal liabilities, potentially leading to HIPAA violations and professional problems for both the trainee and Fran as the supervisor. The trainee's behavior prevents effective supervision and professional development, demanding immediate and structured intervention to ensure compliance with the ethics code.

Fran, guided by the Ethics Code, must address the trainee's problems. First, she must immediately ensure that client records are confidential and address the confidentiality breach, providing training (2.03, 2.05). Second, she needs to implement structured interventions, including increased supervision, goal setting, and regular feedback (4.04, 4.08). If necessary, she must evaluate the trainee's knowledge and revise responsibilities if needed (1.05, 1.06, 4.09). Throughout this process, Fran must follow ethical decision making standards and ensure responsibility (1.03). Additionally, she must be aware of cultural and disability backgrounds (1.07, 4.07).

disorganization that could lead to issues with documentation

formal feedback and continued monitoring

Trainee Doesn’t meet professional standards (late)
Trainee don’t keep an appropriate record of client notes, or supervision hours and doesn’t meet confidentiality requirements
Supervisor shows favoritism (likes the trainee), hasn’t developed an performance improvement plan tailored to the trainees needs

They would need to develop a performance improved program tailored to the trainees needs. Part of this behaviours should be operationally defined, explain documentation type, and set follow up/feedback meetings as well as identify reinforcement

hippa violation, incorrect documentation

remind him of it, review wiht him. Create organzational tools and reminders with deadlines.

Poses the risk of violating privacy of client information (HIPAA)

Establish a plan of action to meet more frequently after outlining specific expectations.

The trainee is unable to keep his items organized and be on time for meetings. This could cause a risk that he may lose paperwork or be late to meetings in the future.

The supervisor should provide feedback to the trainee about his performance and continue to model appropriate behaviors.
